Once the patient declares an internal emotional problem, ask if it is her will to seek therapy. Often, patients have been referred against their will or under a threat by a spouse, boss, or even court authorities. We have no right to do therapy without the patient’s desire and permission. If the patient expresses verbally or nonverbally any reluctance in the opening minutes, ask, “Is it your will to be here?” “Did you come here of your own free will?” “Are you required to come here, or was this your decision?” If a patient does not declare an internal emotional problem or her will to do therapy, you have no right to work with her.
